Enrollment Forms

​

These forms are required before your child can attend Barre City Kids. In addition, I will also need a copy of your child's immunization record. These forms can also be used to make any changes to your child's schedule or file.

Deposit

​

A deposit totaling two weeks of care is required to reserve your child’s spot. This deposit will not be returned if your child(ren) is/are not placed in care by the agreed upon date. The deposit is used for your child’s final two weeks of care.
